The most credentialed of the Khavinson bioregulators, and that is a low bar. Thymalin has decades of Russian clinical use for immune support and a handful of human studies that hint at something real, including a long-term aging trial with a mortality signal. The catch: every study is small, old, or methodologically rough, and nothing modern or Western has replicated any of it. Worth knowing about, not worth building a health plan around.

Would I take it?Interesting history, thin proof.

What it is

Thymalin is a polypeptide complex extracted from animal thymus tissue, not a single synthetic sequence like most of its bioregulator siblings. The thymus trains your immune system’s T cells, and thymalin’s pitch is restoring that function as the gland shrinks with age. In cell work it nudges stem cells toward T-cell markers and dials down inflammatory cytokines. In a rat bone-defect model, it shifted macrophages from the destructive M1 type toward the repair-oriented M2 type. That immune-modulation story is consistent and well-mapped at the bench level.

Among the bioregulators, thymalin and epitalon sit a tier above the rest. Testagen, vesugen, and vilon have little beyond cell and animal work. Thymalin at least has human studies, even if none of them would survive a modern review board without changes.

What the research shows

More human data than any bioregulator on this shelf, and still not much by modern standards:

In 266 older adults followed for 6 to 8 years, mortality was 2.0 to 2.1-fold lower in the thymalin-treated group than in controls. The combination of thymalin plus epithalamin dropped it 2.5-fold.Neuro Endocrinol Lett 2003 · clinical · n=266
In severe COVID-19, hospital mortality was 40.9% with standard therapy, 28.4% with tocilizumab, and 20.6% with added thymalin.Adv Gerontol 2022 · comparative · severe COVID-19
In 32 patients with degree III to IV burns, thymalin shortened hospital stay by 11 days and moved first autodermoplasty forward by 5 days.Vestnik Khirurgii 2001 · comparative · n=32
In 44 children with herpetic stomatitis, thymalin did not produce rapid abatement of acute symptoms.Stomatologiia 1994 · comparative · n=44
No modern placebo-controlled trial designed to quantify adverse-event rates for thymalin exists in the published literature.as of Aug 2026

Realistic protocols

The standard bioregulator protocol applies. Thymalin is run in short courses of intramuscular or subcutaneous injections, typically 5 to 10 mg daily for 5 to 10 days, then a break of several months before repeating. The Russian clinical tradition cycles it once or twice a year, usually in people over 40 chasing immune support or seasonal resilience. There is no pharmacokinetic data to guide timing, so the protocol is tradition, not science.

If you are here for immune optimization and want something with a modern evidence base, you are in the wrong aisle. Vitamin D, zinc, and consistent sleep do more for your immune system than any peptide on this shelf. Thymalin is for people who have read the old studies and decided the signal is worth chasing despite the gaps.

Course5–10 mg/dayIM or sub-q, daily for 5 to 10 days. One course, then stop.
Break3–6 monthsThe standard bioregulator rest period between courses.
Repeat1–2x/yearSeasonal cycling is the most common pattern, often fall and spring.

Side effects

The published studies report almost nothing on adverse events, which is itself a data gap:

anecdotalInjection-site soreness and occasional low-grade fatigue during a course, reported in community logs.
monitorThymalin modulates immune-cell populations. If you have an autoimmune condition, stimulating your immune system is playing with fire. This is not the compound for you.
unknownNo modern safety trial has quantified adverse-event rates. Decades of Russian clinical use is a soft signal, not a safety profile.
