Propranolol
most data2 sources
Propranolol is the cleanest tool on this shelf. It blocks the physical symptoms of anxiety, the racing heart, the trembling hands, the voice that won’t hold still, and it does nothing to the thoughts. For a presentation, an audition, a date where your hands betray you, 10 to 40 mg 30 to 60 minutes before is a genuine fix with decades of safe use behind it. If you want something that changes how you feel inside, this is the wrong compound.
What it is
Propranolol is a beta-blocker, a drug that sits on the beta-adrenergic receptors your adrenaline normally hits and blocks the signal. When adrenaline surges, the heart speeds up, the hands shake, the sweat starts. Propranolol intercepts that cascade at the receptor, so the adrenaline fires but the body doesn’t respond. The anxiety in your head stays exactly where it was. The physical tell disappears.
That makes it a completely different animal from everything else in this column. Pregabalin, gabapentin, baclofen, and phenibut all work centrally, changing how the brain processes anxiety. Propranolol works peripherally, muting the body’s output. No sedation, no euphoria, no dependence, no withdrawal. It is a tool, not a substance, and for the narrow job of situational performance anxiety it has no real competitor here.
What the research shows
The performance-anxiety use is borrowed from propranolol’s long life as a cardiovascular drug. The formal trial base is almost entirely in other indications, migraine and infantile hemangioma chief among them, none of which is why people on this shelf reach for it. The relevant numbers:
Realistic protocols
This is not a daily drug for this use. You take it when you need it, and you leave it in the drawer the rest of the time. One dose, 30 to 60 minutes before the moment, and it lasts a few hours. Start at 10 mg to see how you respond. Most people land between 10 and 40 mg for a single event. If 10 mg barely registers, try 20 next time. If 40 mg handles a keynote, that is your number and there is no reason to chase it higher. The immediate-release tablet is the one you want for this; the extended-release formulation is for daily cardiovascular use and too slow for a pre-event dose.
Propranolol drops your heart rate, and that changes two things about your day. If you lift or run after taking it, your heart rate caps out lower than normal and the session is wasted, so schedule training away from the dose. Alcohol stacks with the blood-pressure drop, so skip drinking on the same day you take it.
Side effects
Propranolol is one of the best-tolerated drugs on any shelf, but it is a beta-blocker and it acts like one:
