Pramiracetam is the sharpest racetam by reputation, with a small handful of old human trials showing real signals: partial reversal of drug-induced amnesia, memory improvement after brain injury, and memory gains in healthy older adults. None has been replicated. If you have tried piracetam and want more focus at a lower dose, pramiracetam is the natural next step. Just know the evidence is a sketch, not a portrait.

Would I take it?Promising signals, nothing replicated.

What it is

A fat-soluble racetam that runs at about a quarter of piracetam’s dose, 1200 mg a day versus 4800. The proposed mechanism: pramiracetam increases high-affinity choline uptake in the hippocampus, fueling acetylcholine synthesis, the neurotransmitter most tied to memory consolidation. Community users describe it as the cleanest nootropic in the racetam line, focused and narrow, without the anxiolytic warmth of aniracetam or the stimulant edge of phenylpiracetam.

What the research shows

The human data is small and decades old, but it is real, and the signals point in the right direction:

In 24 healthy men given 600 mg twice daily for 10 days, pramiracetam partially reduced scopolamine-induced amnesic effects versus placebo in both younger and older age groups.Drugs Aging 1994 · RCT · n=24
In young men with cognitive problems after brain injury and anoxia, pramiracetam 400 mg three times daily improved memory measures versus placebo, especially delayed recall. The improvement persisted during 18 months of open treatment.Brain Inj 1991 · RCT
In 10 patients with probable Alzheimer disease, doses up to 4,000 mg were unlikely to provide symptomatic benefit. Eight improved during dose-finding, but only two reproduced that improvement in replication.Neurology 1991 · enrichment trial · n=10
In 35 healthy older adults with objective or subjective memory loss, the two pramiracetam groups had significantly larger objective memory gains than memory training alone and control.Aging 1996 · open-label pilot · n=35

Realistic protocols

Pramiracetam is fat-soluble, so take it with a meal or a fat source for better absorption. Most people run 400 mg two or three times daily, and the community consensus is that the full effect takes about a week to settle in. Pair it with a choline source: pramiracetam increases hippocampal choline uptake in rat studies, and headaches without supplemental choline are the number-one community complaint.

The effective window is narrower than piracetam’s. The human trials used 600 mg twice daily and 400 mg three times daily, both landing around 1200 mg a day. Going above that range rarely adds benefit and sometimes brings a flat, emotionally blunted feeling that users describe as too clean.

Start400 mg 2x/dayWith meals. Give it a week before judging.
Full dose400 mg 3x/day1200 mg/day, the range the human trials used.
The stack+ choline sourceNon-negotiable. Headaches without it are the top complaint.

Side effects

Mild overall, with one consistent theme:

anecdotalHeadaches without a choline source, the most common community complaint. Add alpha-GPC or CDP-choline and they resolve.
anecdotalEmotional blunting or a flat, over-focused feeling at higher doses, widely reported by users who push past 1200 mg/day.
anecdotalGI irritation on an empty stomach, per community reports. Taking pramiracetam with food fixes this and improves absorption.
unknownNo modern long-term safety study exists. In the one single-dose tolerance study of 12 healthy men at 400 to 1,600 mg, no significant side effects were observed at any dose.
