Piracetam has more human data than almost anything else on this shelf, and the answer is still underwhelming. It works in clinical niches, pediatric breath-holding spells especially, but the dream that piracetam sharpens a healthy brain has never been tested in a controlled trial. It is cheap, safe, and easy to try. Just calibrate your expectations.

What it is

The first racetam, synthesized in 1964, and the compound that gave the world the word “nootropic.” Piracetam modulates AMPA receptors and improves neuronal membrane fluidity, helping damaged or aging neurons communicate better without the obvious kick of a stimulant. It has been prescribed across Europe and Russia for decades for cognitive decline, vertigo, and various neurological conditions. In the community it is the entry point: cheap, subtle, and endlessly debated.

Among the racetams, piracetam is the gentlest and the weakest by dose. Pramiracetam runs at a fraction of the dose for deeper focus. Aniracetam adds an anxiolytic color. Phenylpiracetam adds stimulant energy. They all trace back here.

What the research shows

The evidence is voluminous and underwhelming for healthy cognition. It tells a clear story: piracetam does something, just not reliably what the nootropic buyer is hoping for.

Realistic protocols

Most people start at 800 mg two or three times a day, looking for a subtle lift in verbal fluidity or recall, the typical piracetam signal. If a lower dose is working, stay there. The full community dose is 4800 mg a day, split into three doses, but more is not automatically better. If nothing registers at that level, piracetam is probably not your racetam, and going higher will not change that.

Pair it with a choline source. Piracetam increases acetylcholine turnover, and the headache every first-timer hears about goes away when you add choline. Alpha-GPC or CDP-choline with each dose handles it. Take piracetam with or without food.

Side effects

Piracetam is one of the safest compounds on the shelf. The issues are minor:
