Dutasteride is the strongest DHT blocker on the shelf, and head-to-head it beats finasteride on hair counts. Reach for it when finasteride is not enough, not instead of trying finasteride first. The deeper DHT suppression that makes it more effective also reaches the neurosteroids your brain relies on, and that tradeoff deserves respect.

What it is

A dual 5-alpha-reductase inhibitor, blocking both type 1 and type 2 enzymes where finasteride only hits type 2. That wider coverage cuts DHT harder and deeper than finasteride can. The extra suppression is why dutasteride grows more hair, and also why it carries more systemic weight: the same pathway that produces DHT also feeds allopregnanolone and other neurosteroids the brain uses for mood regulation and anxiety buffering. Suppressing DHT more completely means suppressing those too, and that is the honest reason dutasteride sits behind finasteride in line.

Dutasteride lingers for weeks, not hours, which changes how you can dose it and how long it takes to clear if you stop.

What the research shows

Head-to-head data against finasteride exists, which is more than most compounds on this shelf can claim:

Realistic protocols

Dutasteride belongs in the conversation after finasteride has had its shot and fallen short, not as the opening move. If you are switching from finasteride, the transition is simple: swap the pill. If you are starting fresh because your loss is aggressive, start at the lower dose and see.

The 0.2 mg dose matched 0.5 mg in a phase 3 trial, so starting low is not underdosing. Because dutasteride clears so slowly, intermittent dosing works: early data suggests three times a week can hold meaningful hair counts. Taking it every other day or three times weekly is a real strategy for managing the systemic load.

Topical dutasteride is in trials, with early phase 2 data showing dose-dependent hair growth. Worth watching, but not widely available yet. Stack with minoxidil for the same reason you would on finasteride: one blocks the cause, the other pushes regrowth.

You cannot donate blood while on dutasteride, and the drug lingers long enough that most blood services will not accept you for months after stopping. If you donate regularly, factor that in before starting.
