Dutasteride vs Minoxidil
8 sources
Different jobs, so the answer is both, in the right order. Dutasteride stops the DHT that is shrinking your follicles, and minoxidil pushes the follicles you still have back into growing. Minoxidil goes on from day one. Dutasteride does not: I start a man on finasteride plus topical minoxidil, and dutasteride takes finasteride’s slot only if a full year falls short. If it has to be one of these two and you are a man, take dutasteride. For women, minoxidil is the pick, and dutasteride stays off the table while pregnancy is possible.
How they differ
The difference that matters is what each one fixes. Pattern hair loss is DHT, a testosterone byproduct, shrinking the follicles on your crown and hairline until they quit. Dutasteride blocks both enzymes that make DHT, so it goes after the cause, and harder than finasteride does. Minoxidil never touches DHT. It is a vasodilator, a blood-vessel opener, and it pushes resting follicles into growing and keeps them there longer. Run minoxidil alone and you are growing hair on follicles DHT is still shrinking. Run dutasteride alone and you keep what you have with some regrowth, but leave the push on the table. If you are a man set on only one of these two, take the DHT blocker, because the cause beats the push. For a woman it is minoxidil alone.
This is oral dutasteride against minoxidil by either route. Topical dutasteride is in trials, not on shelves you can trust yet, so a dutasteride lotion is not a real option today.
They also cost you different things. Dutasteride’s price is hormonal: the deep DHT cut reaches the neurosteroids your brain uses to steady mood, and the sexual side effects run in finasteride’s family. Minoxidil’s price depends on the route. Topical stays mostly on your scalp, and the cost is a daily chore and an itchy scalp. Oral minoxidil swaps the chore for a pill, but it raises your heart rate and grows hair everywhere, not only on your head.
Dutasteride forgives a missed day; minoxidil does not. Dutasteride works on for weeks after a dose and takes months to clear fully, while minoxidil only works while you keep using it, and a few skipped days of topical start costing you. Dutasteride can harm a male fetus, so no woman who could become pregnant should take it or handle a leaking capsule.
What the research shows
No big trial has put these two side by side, so this is two evidence piles from different trials in different people:
That ranking puts oral and topical treatments on one list, the only place these two meet, and it is indirect. It still backs dutasteride as the pick if you run one. Dutasteride is the strongest single tool on the shelf, with a mood signal that earns respect. Minoxidil grows about the same hair by either route, and the oral route bills your heart for the convenience.
How people run them
Run them together, and add them in order. If you are on nothing yet, start finasteride and topical minoxidil and give the pair a year. If the year falls short, swap finasteride for dutasteride the next day, no taper, and leave minoxidil exactly as it was. Change one thing at a time, or you will never know which one moved the needle or caused the side effect.
Dutasteride is one capsule a day, 0.5 mg as the standard or 0.2 mg, which matched it in a phase 3 trial, so starting low is not underdosing. Because it lingers, some people settle on 0.5 mg three times a week to lower the load. Minoxidil is topical 5% on a dry scalp, once or twice a day. Let it dry before bed, wash your hands after, and keep it away from cats, because it is toxic to them. Expect a shed between weeks two and eight; that is resting hairs making way, not the stack failing. Judge the pair at six months, not six weeks.
Two pills and no scalp routine, oral dutasteride plus oral minoxidil, is the popular Reddit setup for people who hate topicals. It works, but it puts both drugs’ costs on your whole body; keep oral minoxidil at 2.5 mg or below, because the heart load scales with dose and the hair benefit plateaus past that. Before dutasteride specifically: blood services will turn you away for about six months after your last capsule, and a partner who is or could be pregnant should not handle a leaking one.
Side effects, side by side
Two unrelated side profiles, which is part of why they stack cleanly:
