Sermorelin vs Tesamorelin
7 sources
Tesamorelin, if visceral belly fat is the target. It is the one with an approval and real trials behind it, one fixed pin a day, and the catch is that the fat it takes comes back when you stop. Sermorelin is the older, gentler one: it does release your GH, but it asks for two or three pins a day and has no modern randomized adult trial to its name. For general GH-axis work like recovery, sleep, or a slow recomp, I’d skip both and run CJC-1295 with ipamorelin. If a clinic offers only these two and belly fat is not the issue, sermorelin with ipamorelin is the gentler road.
How they differ
The difference that matters is staying power, and it decides your week. Both are GHRH analogs, copies of the signal your hypothalamus sends to make your pituitary release growth hormone, which makes both growth-hormone secretagogues: your body makes the GH, the shot only asks for it. Sermorelin is the first 29 amino acids of natural GHRH, left as nature built them, so the enzyme that chews GHRH up clears it fast and you chase it with two or three pins a day. Tesamorelin is the full 44-amino-acid molecule with a cap on the front that the enzyme cannot bite, so one daily pin carries it.
The second difference is what the evidence covers. Tesamorelin’s trials were in people with HIV whose medication had packed fat deep around their organs, and the trials measured that fat shrinking on scans. Sermorelin’s record is growth-deficient children and diagnostic tests from the 1980s. If you searched for muscle growth, neither one is a muscle builder: tesamorelin’s lean-mass number below comes from HIV patients, and the gains people log on either ride on training and food more than on the peptide.
Either one pairs with ipamorelin, the GHRP partner that turns a GH nudge into a proper pulse, though for that pairing I’d reach for CJC-1295, the GHRH analog that does sermorelin’s job with fewer pins. Both are empty-stomach shots, so both push your meals around the pins. Sermorelin’s extra daily pins are the bigger tax on your day; tesamorelin is one pin, but every day for months, because it only holds the belly fat while you keep running it.
What the research shows
Nobody has run these two against each other, and their trials sit in different people for different jobs:
Tesamorelin’s receipts are real but narrow, and sermorelin’s are old and pediatric. Neither tells you much about a lifter’s recomp, which is why CJC-1295 with ipamorelin stays my default for that.
How people run them
Sermorelin runs at 200–300 mcg sub-q, two or three times a day, on an empty stomach. Pre-bed is the anchor, riding your natural overnight GH surge, and it is the one to keep if you drop to a single daily shot. Tesamorelin is simpler: 2 mg once a day, empty stomach, fixed. The trials used 2 mg flat, so there is nothing to titrate. They dosed in the morning; some people pin it pre-bed instead. Plan on 26 weeks or more, because that is where the data lives.
Switching from sermorelin to tesamorelin is clean: take your last sermorelin pre-bed and start tesamorelin the next morning. No washout; sermorelin is gone within hours. Going the other way, usually to save money once the belly target is met, expect the visceral fat to drift back. Nothing shows sermorelin holds the line tesamorelin drew.
Do not stack them. Both push the same button on your pituitary, and two GHRH analogs buy you more pins, not more GH. If you want the pairing, put ipamorelin next to whichever one you run.
Side effects, side by side
Same class, same mild shape; tesamorelin’s trials simply wrote more of it down:
