Picamilon is gentle, well-tolerated, and easy to run. The catch: the human data is open-label Russian work, mostly in cerebral ischemia patients, with no placebo arms, no blinding, and nothing that tells you what it does for a healthy brain. If you want a mild anxiolytic that nudges cerebral blood flow, it is a low-risk experiment. If you want real receipts, semax and noopept have more human data behind them.
What it is
Picamilon is niacin bonded to GABA, designed to cross the blood-brain barrier, which GABA alone cannot do. Once across, picamilon cleaves: the GABA half delivers a mild anxiolytic and neuroprotective signal, the niacin half dilates cerebral vasculature. Russian medicine prescribed it for decades for circulatory complaints and mild anxiety. On the nootropic shelf it sits in the gentle, well-characterized corner, more circulatory support than cognitive sharpener, with a safety record from clinical use abroad and no controlled evidence for healthy-brain enhancement.
What the research shows
The evidence base is real but uncontrolled. Two recent Russian studies in chronic cerebral ischemia patients carry the main numbers, both open-label, and both used a parenteral loading phase before the oral course the reader would actually run. A separate four-week study in swimmers is the only non-ischemia human data.
Realistic protocols
Picamilon is straightforward to run. Most people take 50 mg two or three times a day and adjust from there. The Russian clinical studies used oral 50 mg three times daily for 60-day courses, though both started with an injectable loading phase that a self-experimenter will not replicate. If you feel it, you feel it within a few days: a mild background calm, slightly less mental friction, sometimes a gentle warmth from the niacin component. If nothing registers after two weeks at a working dose, more will not change that.
One thing to know before sourcing: US supplement labels for picamilon are unreliable, with tested products varying from 2.7 to 721.5 mg per serving. If you are buying a supplement rather than raw powder, verify the actual content or you have no idea what dose you are running.
Take it with food to smooth any niacin flush. Because picamilon is GABAergic, skip alcohol and sedatives while you are running it. Consider running it in courses rather than indefinitely, since the clinical data was all course-based.
Side effects
One of the mildest profiles on the shelf:
