Ketoconazole shampoo will not save your hairline on its own. What it does is keep your scalp healthy, knock down inflammation and yeast, and add a mild local anti-androgen effect for nearly zero cost or risk. In the one study that ranked hair-loss regimens, finasteride plus ketoconazole beat finasteride alone. If you are running a real stack, adding this is free money.

What it is

An antifungal shampoo that kills Malassezia yeast, the main driver of dandruff and seborrheic dermatitis, with a mild anti-inflammatory and anti-androgen effect at the scalp as a bonus. Ketoconazole shampoo is not a hair-loss treatment in the way finasteride or minoxidil are. It is the supporting player that creates a healthier scalp environment for the real treatments to work in, and may contribute a small direct effect on DHT at the follicle. Early comparisons hinted at a modest direct hair benefit, but no controlled trial has confirmed it on its own.

Realistic protocols

This is the easiest protocol in the category. Use 2% ketoconazole shampoo two to three times a week in place of your normal shampoo. Lather it on, leave it on your scalp for three to five minutes so the active ingredient has contact time, then rinse. That is the entire protocol. Once your scalp is in good shape, dropping to once a week for maintenance is fine.
