CMS-121 is early but genuinely interesting. A synthetic fisetin derivative with a clean mechanism, strong rodent results across Alzheimer’s, Huntington’s, and aging models, and a phase 1 preprint that cleared safety in healthy volunteers. That is more runway than most compounds on this shelf ever build. It is also still preclinical for any real endpoint. Nobody has tested whether it helps a person think better or age slower, and there is no approved product, no finished dosage form, and no source with a COA behind it. Worth tracking, not worth running yet.

What it is

CMS-121 is a synthetic derivative of fisetin, the flavonoid found in strawberries and marketed as a longevity supplement. The Salk Institute built it to be more potent and more drug-like than fisetin itself, and the mechanism turned out to be fatty acid synthase inhibition, redirecting lipid metabolism in a way that reduces brain inflammation and oxidative damage. In aging and neurodegeneration mouse models, it consistently protects cognition, slows motor decline, and improves metabolic markers.

Fisetin itself has a following in the anti-aging community, mostly around senolytic dosing. CMS-121 is the pharmaceutical-track version of that idea, designed for oral bioavailability and aimed at neurodegeneration rather than general senescence. On the nootropic shelf it sits among the compounds with real mechanistic depth and no human proof yet.

What the research shows

Rodent work across multiple disease models, plus a phase 1 that establishes human tolerability but nothing else:

Realistic protocols

CMS-121 has no approved product or finished dosage form. A phase 1 preprint cleared it for safety, which means doses up to 900 mg daily for a week did not cause serious problems in healthy volunteers, but no efficacy dose has been established and no patient has been treated in a trial. The phase 1 tells you CMS-121 is tolerable, not that it works at any particular dose. If fisetin’s neuroprotective angle appeals to you, fisetin itself is available, cheap, has a longer safety record, and carries its own senolytic and anti-inflammatory story, though its clinical evidence for cognition is thin. CMS-121 is the one to watch for trial results, not the one to self-experiment with today.
