Agomelatine is the right tool for the right problem: when poor sleep is tangled with low mood and a broken circadian rhythm, it fixes both at once by resetting the clock and lifting the floor. The mood data is solid. The sleep data is subjective: better self-reported quality, but no significant improvement in objective sleep time. The catch is your liver. Agomelatine needs baseline liver function tests and repeat checks for the first year, which makes it a more supervised compound than trazodone. Worth it for the person it fits. If you just need to fall asleep, trazodone is simpler.
What it is
A melatonin-receptor agonist that also blocks serotonin 5-HT2C receptors: two mechanisms in one pill. The melatonin arm resets your circadian clock, pulling sleep onset earlier and stabilizing rhythm. The 5-HT2C antagonism lifts mood and increases dopamine and noradrenaline release in the prefrontal cortex, which is why it is approved as an antidepressant, not a pure sleep aid. The combination is the point. Where trazodone just sedates and lemborexant just blocks the wake signal, agomelatine resets the underlying timing and treats the mood that was disrupting it.
Outside Europe and Australia it is harder to find. It was never brought to market in the US. That does not make it less real, just less accessible.
What the research shows
Multiple meta-analyses across depression, sleep, and anxiety, with safety tracked in large cohorts:
One gap: in 123 patients who had not responded to an SSRI or SNRI, adding agomelatine did not improve depression scores over placebo. The wins are against placebo alone.
Realistic protocols
Take it at bedtime, every night, at the same time. Consistency matters here because the circadian mechanism works on rhythm, and irregular dosing undermines it. Start at 25 mg. If the mood response is not enough after two weeks, go to 50 mg. Most people stay at 25 mg for sleep alone.
The liver check is non-negotiable: get a baseline, repeat at roughly 3, 6, 12, and 24 weeks. If transaminases go past three times the upper limit, stop. The actual incidence is low and comparable to SSRIs, but the monitoring exists because the first cohorts flagged it and regulators enforced it. Do not skip the labs.
Smoking matters. Agomelatine is cleared through CYP1A2, and heavy smokers burn through it faster and may need the higher dose. If you quit smoking while on it, your levels will rise. Alcohol: not the emergency it is with the GABA sleep drugs, but it worsens drowsiness and is best avoided on treatment nights.
When you stop, taper over a week out of good practice. Agomelatine carries less withdrawal risk than most antidepressants, which is a genuine advantage if you have ever tried to come off an SSRI.
