Ipamorelin vs Tesamorelin
10 sources
For general GH-axis work, meaning recovery, sleep, and a slow recomp, my pick is ipamorelin, paired with CJC-1295. Tesamorelin wins one job: visceral belly fat, where it has phase 3 trials and an FDA approval and ipamorelin has nothing. It works the other half of the axis, the GHRH slot CJC-1295 fills, so the real choice is the ipamorelin stack against tesamorelin, and the deciding question is whether deep belly fat is your target. If it is, run tesamorelin daily and plan to keep running it.
How they differ
The biggest difference is receipts. Tesamorelin has phase 3 trials, an approval, and a measured result on the deep abdominal fat that wraps your organs. Ipamorelin has a clean mechanism and no body-composition trial at all. That gap is the whole case for tesamorelin, and it only covers that target: the trials were in people with HIV, and outside that lane its recomp case is borrowed from the same mechanism CJC-1295 runs on.
Mechanically, they sit on opposite sides of the axis. Tesamorelin is a GHRH analog, the release signal your hypothalamus sends. Ipamorelin is a secretagogue that hits the ghrelin receptor and gets your pituitary to release a GH pulse on the spot. That makes them partners as much as rivals: some people run tesamorelin as the GHRH half with ipamorelin on top. There is no reason to run tesamorelin and CJC-1295 together, because they fill the same slot.
For fat loss, tesamorelin is the one with numbers. For muscle, neither builds much you’d see next to training and food; the ipamorelin stack is the cheaper, lower-commitment way to run the GH axis for recovery, and tesamorelin is a daily routine for months that undoes itself when you stop. None of that changes for women; the choice is the target, not who is pinning.
What the research shows
No trial has put the two side by side, and their evidence is not even the same kind. These are different studies in different people, one set in HIV patients, the other in animals and healthy volunteers:
How people run them
Tesamorelin is 2 mg sub-q once a day, flat, no titration, because that is the dose the trials used. The trials dosed in the morning; empty stomach either way. Plan on six months or more, and plan on it staying daily, because the belly fat comes back when you stop. That makes it a routine, not a course.
The ipamorelin stack is 200–300 mcg of ipamorelin with 100 mcg of no-DAC CJC-1295 in one syringe, pre-bed, on an empty stomach, adding a morning shot if you want more. It is the lower-stakes option: miss a night and you lose a night, not the progress.
Switching from the stack to tesamorelin, drop CJC-1295 and start tesamorelin the next morning; no washout, since they share a slot. Keep ipamorelin pre-bed if you like, as the trigger tesamorelin doesn’t provide. Coming off tesamorelin, go back to the full stack the same night, and expect the visceral fat to drift back, because the stack has no receipt for holding it.
Side effects, side by side
The same raised-GH signals on both; tesamorelin’s are trial-documented, ipamorelin’s mostly firsthand:
