The most proven SERM in the toolkit. Tamoxifen has decades of clinical use, breast cancer prevention data that nobody else touches, and a track record in PCT that makes it the default for a reason. It restarts your axis, it catches early gyno, and it does both reliably. Enclomiphene is the modern smart pick with a cleaner side profile, but tamoxifen is the one with 30 years of receipts behind it. The catches are real but manageable: hot flashes, a small clotting risk on long runs, and CYP2D6 genetics that may affect how well you metabolize it. For PCT, pair it with on-cycle hCG and you have the backbone of most restart protocols.
What it is
Tamoxifen is a SERM, a selective estrogen receptor modulator, that blocks estrogen in some tissues and mimics it in others. At the breast it is an antagonist, which is why it prevents and reverses gynecomastia. At the hypothalamus it blocks estrogen’s negative feedback, which drives LH and FSH up and restarts testosterone production. At bone it acts as a mild estrogen agonist, which is actually protective. That tissue selectivity is the design: you get the anti-estrogen effect where you want it without wiping estrogen’s benefits everywhere else.
The metabolism matters. Your body converts tamoxifen into endoxifen, its most potent active metabolite, through the CYP2D6 enzyme. If your CYP2D6 is slow, genetically or because another drug is blocking it, you make less endoxifen. About 7 to 10% of people are poor metabolizers. Whether lower endoxifen levels actually translate to worse outcomes is still contested: the largest prospective study, CYPTAM with 667 patients, found no association between endoxifen concentration or CYP2D6 genotype and relapse-free survival. The pharmacology is clear, the clinical consequence is not.

Realistic protocols
Tamoxifen is the anchor of the standard PCT protocol, and for good reason: it reliably drives LH and FSH back up, it protects against gyno while estrogen rebounds, and you can dose it simply. The usual PCT run is 20 mg per day for four to six weeks, starting after your last compound has cleared, roughly two weeks after a long-ester injectable or immediately after orals. Some older protocols front-load at 40 mg for the first week, but there is no strong reason to: 20 mg saturates the receptor, and the higher dose just brings more hot flashes.
If you ran hCG on cycle, your testes are already primed and recovery is faster. If you did not, consider a short hCG bridge, 1000 to 1500 IU every other day for 10 days, before starting tamoxifen. Tamoxifen tells your brain to send the restart signal, but if your testes spent months offline and atrophied, they need hCG to wake up before the signal means anything.
For gyno prevention on cycle, 10 to 20 mg daily catches most early signs. If you already have a lump, tamoxifen can reverse early-stage gyno, but raloxifene outperforms it for established tissue.
Two drug interactions to know. SSRIs like paroxetine and fluoxetine block CYP2D6 and cut your endoxifen levels, making tamoxifen less effective. If you are on one, either switch the antidepressant or switch the SERM. Grapefruit juice hits CYP3A4, the other enzyme in the pathway, and is worth avoiding while you are on tamoxifen.
