Nefiracetam
guinea pig5 sources
Nefiracetam has an interesting mechanism and real human trials, which puts it ahead of most racetams on paper. The problem is testicular toxicity in dogs at high doses, with sperm motility and morphology both hit. The human trials tested post-stroke depression and apathy and came back weak and mixed. If you want a racetam, aniracetam and piracetam are cleaner picks with longer track records. If you want this one specifically, read the dog data first.
What it is
Nefiracetam is a fat-soluble pyrrolidone nootropic, part of the racetam family but with a wider mechanism than most of its siblings. Where piracetam and aniracetam work primarily through AMPA modulation, nefiracetam also potentiates NMDA currents, the glutamate receptor type tied to learning and plasticity, through a protein kinase C pathway and enhances nicotinic acetylcholine receptor signaling, the system that supports attention and memory encoding. That triple-channel profile across glutamate and cholinergic systems is what drew clinical interest.
That mechanistic breadth is what drew clinical interest for mood and cognition after stroke, where multiple neurotransmitter systems are disrupted at once. It also means the compound touches more biology than a simple racetam, which cuts both ways when the safety data has gaps.
What the research shows
The human trials exist but underwhelm. The animal safety signal is the finding that follows you home:
Realistic protocols
If you run nefiracetam despite the toxicity flag, most people settle at 600 to 900 mg daily, split into two or three doses. The clinical trials used 600 to 900 mg daily in post-stroke patients. The racetam community generally pairs it with a choline source like alpha-GPC to support the acetylcholine demand, the same pairing used across the racetam family. Fat-soluble, so take it with food. The compound clears quickly, which is why splitting doses matters for steady levels.
The toxicity question: the dog seminiferous atrophy appeared at 180 mg/kg and above. A 70 kg person taking 800 mg daily sits at roughly 11 mg/kg, well below that line. But the margin is not so wide that you ignore it, especially with no long-term human reproductive safety data. If you are a man concerned about fertility, skip this racetam.
Side effects
The racetam side profile is generally mild, but nefiracetam carries a flag the others do not:
