Cerebrolysin has more clinical data behind it than almost anything else on the nootropic shelf, and most of that data is about brain injury recovery, not cognitive enhancement. The stroke meta-analyses show consistent short-term neurological gains, but the functional-independence numbers do not reach significance, and the Cochrane safety signal is real. For healthy-brain optimization the evidence is thinner still, the effect is harder to measure, and you are doing intramuscular injections of a porcine peptide mix on a protocol borrowed from neurology wards. I would run a course for recovery after a neurological hit, supervised, before I would run one for general sharpening. If the injectable peptide commitment is too much, the racetams are a lighter on-ramp with lower stakes.
What it is
Cerebrolysin is an enzymatic digest of pig brain tissue, yielding a standardized mix of small peptides and free amino acids that cross the blood-brain barrier. The mix mimics the action of endogenous neurotrophic factors, supporting neuronal survival, synaptic plasticity, and recovery after injury. It has been used clinically for decades in Austria, where it was developed, and across Russia, China, and much of Southeast Asia, primarily for stroke recovery, vascular dementia, and Alzheimer’s disease.
On this shelf it sits in a different weight class from the racetams and research chemicals. The data is real, it has been through Cochrane reviews and multiple meta-analyses, and the clinical protocols are established. The nootropic community borrows those protocols at lower doses for general cognition, which is reasonable but less evidence-backed than the disease use. Cerebroprotein hydrolysate is the related product family, though formulations labeled similarly can differ materially in composition.

The evidence base here is unusually deep for a compound without FDA approval. Multiple Cochrane reviews, meta-analyses, and individual RCTs across stroke, dementia, and brain injury. The pattern: consistent short-term neurological gains, less convincing long-term functional independence.
Realistic protocols
The clinical protocol is courses, not continuous use. Cerebrolysin is given in defined blocks, typically 10 to 21 days of daily injections, then a break of one to three months before the next course. This pattern comes directly from the clinical programs and is how the community runs it as well.
For cognitive enhancement rather than disease recovery, most people run the lower end. 5 mL intramuscular once daily for a 10 to 20 day course is the standard nootropic protocol. Some push to 10 mL, which is still well within the range the clinical data covers. The clinical stroke doses run as high as 30 mL daily, but that is IV, supervised, and for acute brain injury. Stick with the IM range for elective use.
The injection volume is real. 5 mL in one shot is a large IM injection, and splitting into two sites is common. Rotate injection sites. The product needs refrigeration and comes in ampules, so sourcing and storage are part of the commitment.
